Teaching NeuroImages: middle cerebral artery aneurysm rupture presenting as pure acute subdural hematoma.

چکیده
A previously well 33-year-old man with no history of trauma or substance abuse presented with poor right eye visual acuity, somnolence, and vomiting several hours after sudden onset of severe, persistent headache. Examination revealed only a right relative afferent papillary defect and subretinal blood on funduscopy (Terson syndrome, figure 1A). Hunt and Hess grade was 3. CT showed right subdural and subhyaloid hemorrhages (figure 1, B and C). Angiography revealed a right middle cerebral artery aneurysm (figure 2). Aneurysm rupture rarely presents as pure acute subdural hematoma. Proposed mechanisms involve direct aneurysm rupture into subdural space, from orientation, adherence to dura, or rupture through subarachnoid space by a superficial or high-pressure bleed.1 Terson syndrome refers to intraocular hemorrhage with aneurysm rupture. Proposed pathophysiology includes retinal venous bleeding from stasis secondary to increased intracranial pressure, or from blood forced into the subarachnoid space and then along the optic nerve sheath into the preretinal space under pressure.2

Subdural Hematomas Following Intracranial Aneurysm Rupture: A Rare Phenomenon.
Acute Subdural Hematoma (aSDH) due to aneurysm rupture and no subarachnoid bleeding are very rare with only 29 cases reported in literature. A 56-year female presented with headache and drowsiness and a previous history of loss of consciousness. Clinical examination revealed a GCS of 14 and a right sided hemiparesis. Workup revealed a pure subdural hematoma due to a middle cerebral artery aneur...
متن کاملNontraumatic Pure Acute Subdural Hematoma Caused by a Ruptured Cortical Middle Cerebral Artery Aneurysm: Case Report and Literature Review
Nontraumatic or "spontaneous" acute subdural hematoma (SDH) is rare, and "pure" acute SDH without subarachnoid hemorrhage (SAH) due to aneurysmal rupture is extremely rare. We report a case of nontraumatic pure acute SDH caused by the rupture of a cortical middle cerebral artery (MCA) aneurysm. A 43-year-old man with no antecedents, except hypertension, presented to the emergency department wit...
متن کاملIntracystic hemorrhage of the middle fossa arachnoid cyst and subdural hematoma caused by ruptured middle cerebral artery aneurysm.
We report a case of a cerebral aneurysm arising from the bifurcation of the left middle cerebral artery that ruptured into a left middle cranial fossa arachnoid cyst, associated with acute subdural hematoma. We discuss the relationships of aneurysm, arachnoid cyst, and subdural hematoma.
متن کاملSubdural Hematoma without Subarachnoid Hemorrhage Caused by the Rupture of Middle Cerebral Artery Aneurysm
Pure subdural hematomas caused by a ruptured intracranial aneurysm are extremely rare. We describe the case of a 42-year-old woman who presented with headache without evidence of head trauma. Magnetic resonance angiography and conventional cerebral angiography revealed a ruptured aneurysm at the right middle cerebral artery bifurcation. The patient underwent surgical treatment and had a good ou...
متن کاملA misleading distal anterior cerebral artery aneurysm
BACKGROUND Aneurysmal rupture causing pure acute subdural hematoma (aSDH) is rare. In the four previously reported cases of distal anterior cerebral artery (ACA) aneurysm resulting in pure aSDH, blood distribution in the interhemispheric (IH) space has systematically incriminated the distal ACA as the source of rupture. We present a misleading case of a distal ACA rupture resulting in convexity...
